Abstract
Objectives: The purpose of the study was to compare healthy cities focusing on their structure and process by three characteristics of Healthy City: urban/rural, the tenure of healthy city program, and the location of healthy city team.
Methods: A self-administered survey was conducted between November 18 through December 12, 2016. Fifty eight out of 83 healthy cities participated in the survey. The indicators in each of the structure and the process domains were standardized and summed to produce the domain scores. Two sampled t-test was used to compare the structure score and the process score by the three regional variables.

Results: Rural areas had lower structure scores than urban areas. Nither the structure scores nor the process scores were not different between those cities that began healthy city program before 2011 and those that began in 2011 or after. Both structure and process scores were higher where healthy city team was located outside of public health center than where healthy city team was in the public health center.

Conclusions: Tailored supports are needed for rural areas and those cities where the healthy city team is located in their public health center. Capacity building programs are generally needed including the starting group of healthy cities.
